Thank you for participating in Pradko, Gallagher, and Slanec's Referral Program. Whether you have been referred by a patient or are an existing patient who wants to refer someone to our office, please fill out the form below.
From now through March 2022, any existing patient who refers someone new to our practice (any office) will be entered to win a "Spring Staycation" at the historic St. Clair Inn, complete with breakfast and dinner.
PGS patients will be entered in the contest ONLY IF the person they referred makes an appointment at a PGS office between November 2021 and March 2022, and becomes a new patient.

* Winner will be randomly drawn at the end of March 2022. A $600 gift certificate to the St. Clair Inn in St. Clair, Michigan will be awarded to one PGS Patient. The Gift Certificate should cover a one-night stay and breakfast and dinner for two persons. Valid March 2022 - December 2022. Winner will be responsible for booking their own accommodations, dinner, and breakfast reservations. Winner must accept the prize in-person and their local PGS office and agrees to have their picture taken for marketing purposes. If Covid-19 restrictions or other issues beyond our control occur, PGS will issue a prize with a similar value.
